By completing and sending this form, you are ordering the support services of EXAMION GmbH, Erich-Herion-Str. 37, 70736 Fellbach, Germany, on the terms and conditions set out below:

For the support services of the company EXAMION GmbH there are costs of 38 euros for every 1/4 hour started. You will be billed for these costs plus the applicable VAT. We cannot rule out that, despite our best efforts, a technician may be required to visit you, which may lead to additional costs.

If and insofar as the support services are part of an applicable warranty, however, no costs will be incurred for this. Any invoice already issued will be corrected or credited in this case.

This declaration is valid once for the support case.
